Acknowledgment of Risk

I understand axe throwing involves inherent risks including but not limited to cuts, slips, falls, and equipment malfunction. I voluntarily choose to participate and assume all risks, whether known or unknown.

WATL Rules & Safety Compliance

I agree to follow all Screaming Axes safety rules and all applicable World Axe Throwing League (WATL) rules and standards, and to obey all staff instructions and posted signage. Failure to comply may result in removal without refund.

Release of Liability (California)

To the fullest extent permitted by California law, I release Screaming Axes, LLC, its owners, members, employees, agents, contractors, volunteers, venue partners, and affiliates from any and all claims arising from participation, including negligence.

Medical Authorization

If injured or in an emergency, I authorize Screaming Axes staff to seek medical attention on my behalf (or on behalf of the minor) and accept financial responsibility for such care.

Audio / Video Recording Consent

I understand Screaming Axes activities and events may be recorded (audio/video) for safety, security, training, documentation, and promotional purposes. By signing, I consent to being recorded and grant permission to use recordings without compensation, unless revoked in writing for promotional use.

By signing below, I acknowledge that I have read and understand this waiver, I accept responsibility for following all rules and staff instructions, I assume the risks, I release Screaming Axes, LLC as stated above, I authorize emergency medical care if needed, I consent to recording, and I agree my electronic signature is legally binding (E-SIGN/UETA).
