ASSUMPTION OF RISK / RELEASE / INDEMNITY (CALIFORNIA)

I understand axe throwing is hazardous and involves risk of serious injury, property damage, or death.

Assumption of Risk: I voluntarily assume all risks, known and unknown.

Release (Including Negligence): I release Screaming Axes, LLC and related parties, including for negligence, from claims arising from participation.

Indemnity: I agree to indemnify and hold harmless the Released Parties from claims arising from my acts or omissions.

Video/Audio Recording Notice: I understand Screaming Axes may record video and/or audio during events for safety, training, incident documentation, and promotional purposes. I consent to being recorded and to the use of my likeness and voice without compensation where permitted by law.

Medical: I authorize emergency medical care if needed and accept financial responsibility for costs.

CA Civil Code §1542: I waive unknown claims.

Governing Law: California law, venue San Bernardino County.
