Acknowledgment of Risk

I understand axe throwing involves inherent risks including but not limited to cuts, slips, and equipment malfunction. I assume all risks and agree to follow all safety rules and staff instructions.

Release of Liability

I release Screaming Axes, LLC, its owners, agents, and affiliates from any and all claims arising from participation, including negligence to the fullest extent permitted by law.

Medical Authorization

If injured or in an emergency, I authorize Screaming Axes staff to seek medical attention on my behalf and accept financial responsibility for such care.
